As an Ontario health care provider, we handle personal health information in accordance with Ontario's Personal Health Information Protection Act (PHIPA), and other personal information in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA).

Information you provide directly — such as your name, email address, phone number, and how you heard about us when you submit a form — and, if you become a patient, personal health information collected in the course of providing care.
